To register with us you need to be a resident within our practice area. You need to have completed form GMS2 that is available from reception. You will also need to provide proof of address.
You will be asked to make an appointment for a new patient health with one of our Practice Nurses, it is only when you attend this appointment that we accept the completed registration form and register you as a patient of this practice.
It is important that you attend this appointment as it enables us to record any past medical history, medication you take and note any allergies you may have.
Once this registration process is complete we will make the necessary arrangements to ensure that your medical records are sent to us from you previous doctor.
